User Research Consent Form

As part of our efforts to gain a deeper understanding of our users, we would like to observe and record our interview session with you.
At times, we may take notes on your responses, or may ask you to elaborate on or clarify your statements.

As part of the research study you are participating in, a video recording may be made of your responses during your participation in this interview. Your consent is required for the following uses of these video recordings:

1. The video can be studied by CarEdge (Auto Advocate Inc) for use in regards to this study.
2. The video can be reviewed in private presentations to members of the project team.

This is completely voluntary and up to you. In any use of the video recordings, your full name will not be identified. You must check the "I agree" consent box below to indicate your consent to these terms.
